All DRIVINGHEALTH^® INVENTORY including UFOV^® licenses (both unlimited and per-use) are valid for one (1) year from purchase date.

Electronic Payment

If you wish to use electronic payment, either to order the DRIVINGHEALTH^® INVENTORY including UFOV^® on portable media or to open a pay-per-use account to conduct screening via the Internet, select the “Order Online” button. You will then be redirected to PayPal. For pay-per-use customers, a confirmation of your order with account access and login information will follow. Customers purchasing the software on USB flash drive will receive an order confirmation via email.

Check or Money Order Payment

If you wish to mail a check or money order to purchase the DRIVINGHEALTH^® INVENTORY including UFOV^®, select the “Order by Mail” button. Enter all requested information, then print the order form and return it with your payment (in U.S. dollars) to: TransAnalytics Health & Safety Services. Customers purchasing the software on USB flash drive will receive the drive via courier to the street address (not a P.O. Box) provided on your order form.
